I need long distance + computer + reading multifocal glasses

I need long distance + computer + reading multifocal. The optometrist said the glasses would normally have the long focal distance driving above middle, the 500mm focal distance computer finest print magnification from middle to low, with 250mm focal distance reading at very bottom. Do you have any idea which company can supply that type multifocal prescription glasses?
